The information contained in the Partnership for Breast Care (“PBC”) website including any and all information from links to/from other websites (together, the “Website”), is not meant to replace a visit or discussions with your health care provider related to any concerns or questions you may have related to your breast health. The PBC has provided this Website as a source of general, summary information on breast health, breast problems and breast cancer. The information provided on the Website should be used only for informational purposes and should not be used as a supplement to your health care provider’s advice. The opinion of your health care provider should take precedence over any information in this Website. Never delay or disregard medical advice or seeking medical treatment because of information you have read on this Website.
The PBC does not recommend or endorse any specific tests, procedures or products mentioned on the Website. Reliance on any information provided by PBC on the Website is solely at your own risk. PBC assumes no liability or responsibility for any damage or injury to persons or property arising from any use of any product, information, idea, or instruction contained in the material provided to you including without limitation material provided on the Website. PBC reserves the right to change or discontinue at any time, at its sole discretion, any aspect or feature of this Website.
Due to the nature of the Internet, there is a possibility that unsecure (unencrypted) email could be intercepted and read by third parties. For this reason, PBC asks that you do not send any personal and/or confidential information, such as, without limitation, social security numbers, through PBC email links on the Website. Partnership for Breast Care (PBC) Privacy Policy and Agreement
Your privacy is important to the PBC. PBC understands that your breast health is a very personal, private subject, and PBC wants you to feel as comfortable as possible visiting the PBC Website and using its services. This Privacy Policy and Agreement ("Privacy Policy") will tell you what information PBC collects about you and about your use of the Website. It will explain how PBC protects that information and what choices you have about how it is used. If you access a third party site from a link on the PBC Website, any information you reveal on that site is not subject to this Privacy Policy. The following are the articles of our Privacy Policy (the “Articles”) that describe the information we collect from or about you; ways we disclose, use and protect that information; choices you have, and how you can update your information or contact us with questions. We hope that reading the Articles gives you a clear idea of how we manage information about you.
ARTICLE 1: INFORMATION COLLECTED ABOUT YOU
A. Information Collected by PBC or on PBC's Behalf
Personally Identifiable Information (the “Personal Information”) is information that can be used to identify or contact you. When you visit our Website, PBC collects only the Personal Information you provide to us. We only collect the Personal Information that you provide to us in one general and distinct way: When you choose to participate in surveys or send emails to PBC.B. Children
We are committed to protecting the privacy of children. This Website is not designed or intended to attract children under the age of 13. We do not collect Personal Information from any person which we have actual knowledge that said person is under the age of 13.ARTICLE 2: DISCLOSURE OF YOUR PERSONAL INFORMATION
Except as set forth in this Article 2 or as specifically agreed to by you, PBC will not disclose any Personal Information it gathers from you on our Website. Notwithstanding the foregoing, we may release Personal Information to third parties: (1) to comply with valid legal requirements such as a law, regulation, search warrant, subpoena or court order; or (2) in special cases, such as a physical threat to you or others. In the event that we are legally compelled to disclose your Personal Information to a third party, we will notify you, unless doing so would violate the law or court order.A. Disclosure to PBC Subsidiaries
PBC may disclose Personal Information to Hartford Hospital and participating physician groups, including without limitation, board members, officers, and employees. Any Personal Information provided to such subsidiaries of PBC will be treated by those subsidiaries in accordance with the terms of this Privacy Policy.
B. Disclosure to PBC Operations and Maintenance Contractors
PBC operations and maintenance contractors (the “Contractors”) may have limited access to your Personal Information in the course of providing products or services to PBC. These contractors may include vendors and suppliers that provide us with technology, services, and/or content related to better operation and maintenance of the Website. Unless you have opted out of receiving emails and newsletters from PBC, the Contractors also may have access to your email address to send newsletters or special promotions to you on PBC’s behalf or to send emails to you for purposes such as conducting market research on PBC’s behalf. Access to your Personal Information by the Contractors is limited to the information reasonably necessary in order for the Contractor to perform its limited function for PBC. We also contractually require that our operations and maintenance contractors: (1) protect the privacy of your Personal Information consistent with this Privacy Policy, and (2) do not use or disclose your Personal Information for any purpose other than providing us with such products and services.
C. Disclosure to Linked Sites
For your convenience there are links to websites operated by companies other than PBC ("Third Party Websites"). These links to Third Party Websites may be found under other resources in the Website; referenced within Website content; or placed beside the names or logos of entities other than the PBC on the Website. PBC does not disclose your Personal Information to these Third Party Websites without obtaining your consent. PBC does not endorse and is not responsible for the privacy practices of these Third Party Websites. If you choose to link to one of these Third Party Websites, you should review the privacy policy posted on such Third Party Website to understand how that Third Party Website collects and uses your Personal Information.D. Disclosure of Aggregate Information
PBC may provide to certain third parties non-personal information about you that does not allow the third party to identify or contact you and that is combined with the non-personal information of other users of the Website (the “Aggregate Information”). For example, we might inform third parties regarding the number of users of the Website and the activities they conduct while on the Website. Also, we might inform a pharmaceutical company (that may or may not be an advertiser on the Website) that "30% of our users live east of the Mississippi" or that "25% of our users have tried alternative medicine." Depending on the circumstances, we may or may not charge third parties for the Aggregate Information. We also may not limit the third parties' use of the Aggregate Information, except that we do require third parties to whom we disclose Aggregate Information to agree that they will not attempt to make the Aggregate Information personally identifiable by combining it with other databases or otherwise.
ARTICLE 3: YOUR PRIVACY CHOICES
We ask for contact information, including without limitation, name and email address (the “Contact Information”) if you wish to subscribe to various newsletters. If you so choose, you can unsubscribe from such newsletters very easily. When you have received a newsletter you wish to stop, click on the "reply" button in your mail program, then type in the word "UNSUBSCRIBE" in the "Subject" field and click “send”. PBC Customer Service will unsubscribe you from that newsletter in two to three business days.
This privacy statement applies only to the PBC Website. The PBC Website does contain links to other sites. Once you enter another website (whether through an advertisement, service, or content link), be aware that PBC is not responsible for the privacy practices of such other sites. We encourage you to look for and review the privacy statements of each and every website that you visit through a link or advertisement on PBC's Website or any site that collects Personal Information from you.
ARTICLE 4: UPDATING YOUR PERSONAL INFORMATION AND CONTACTING PBC
You can contact PBC in order to (1) delete your Personal Information from PBC systems, (2) update the Personal Information that you have provided to PBC, and (3) change your preferences with respect to marketing contacts or other activities, by emailing PBC at pbc@harthosp.org. Such changes will not have any effect on other information that PBC maintains.
You should be aware that it is not technologically possible to remove each and every record of Personal Information from our servers that you have provided to PBC. The need to back-up PBC systems to protect information from inadvertent loss means that a copy of your Personal Information may exist in a non-erasable form that will be difficult or impossible for PBC to locate. Nevertheless, upon receiving your request, all Personal Information stored in the databases that can be located and deleted, will be deleted.
PBC will notify you by email of any significant changes concerning PBC’s use of your Personal Information and obtain your "opt-in" consent to any significant new uses of your Personal Information if it was collected from you prior to a significant change in the Privacy Policy. PBC may also make non-significant changes to the Privacy Policy that generally will not affect PBC’s use of your Personal Information and PBC generally will not notify you of these non-significant changes. You should check the Privacy Policy from time to time for any such non-significant changes. If you do not agree to the terms of this Privacy Policy or any revised Privacy Policy, please exit the site immediately.
